Advance Care Planning and Life Transition Management Webinar

Older adults with multiple chronic conditions complicated by other risk factors, such as deficits in activities of daily living or social barriers, experience multiple challenges in managing their healthcare needs, especially during episodes of acute illness. Identifying effective strategies to improve care transitions and outcomes for this population is essential. Effective care coordination and transition management are essential to delivering patient centered, quality care. Those who provide these services must be trained and qualified to meet patients' social service and medical needs.

Learning Outcome:

After successfully completing the course, the learner will have a deeper understanding of their role in the continuum of care and will self-report an increase in self-confidence on a Likert Scale of (1-5) in adopting at least two strategies that will allow them to become more effective in their role within the continuum of care for their patients.

Eighty percent of the learners will report that they have confidence on a Likert scale of (1-5), in adopting at least two strategies that will allow them to become more effective in their role within the continuum of care.

Successful completion of the course is defined as attendance for 100 percent of the learning session and a completed course evaluation.

Nurse managers, nursing coordinators, assistant nurse managers, staff nurses, chief nursing

officers, vice presidents of patient care, assistant vice presidents of nursing, administrative

directors, directors of nursing, administrative (house) supervisors, new ONL NJ mentors and mentees, Nursing Workforce Environment Staffing Council (NWESC) members, university deans, nurse educators, nurse researchers and post-acute leaders, nursing home administrators and assisted living administrators, care management and social workers.

Continuing Nursing Education Credits:

This activity has been submitted to the American Association of Critical-Care Nurses for approval to award contact hours. The American Association of Critical-Care Nurses is Accredited as an approver of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.
