Overview
Barriers to equitable access and appropriate care coordination stand in the way of achieving equity in health care and delivering high-quality care to all patients. Notable advances in prevention, screening, and the development of effective treatments have contributed to improvements in outcomes for patients across the continuum. However, the benefits of these advances are not shared equally, and great disparities and biases persist in outcomes by race/ethnicity, socioeconomic status, sex, and geography. This program will offer nurse leaders key strategies and insights to improve transitions of patient care across the continuum.
Learning Outcome: At the conclusion of this program, participants will recognize the barriers to equitable access and appropriate care coordination and its importance in improving transitions of care across the continuum. The learner will be able to identify one strategy to improve transitions of care, for a selected population, that will be transferable to their practice and/or organization.
Successful completion of the webinar is defined as attendance for 95 percent of the webinar learning session and a completed course evaluation.
Target Audience: Nurse managers, nursing coordinators, assistant nurse managers, staff nurses, chief nursing officers, vice presidents of patient care, assistant vice presidents of nursing, administrative directors, directors of nursing, new ONL NJ mentors and mentees, Nursing Workforce Environment Staffing Council (NWESC) members, university deans, nurse educators, nurse researchers and post-acute leaders, nursing home administrators and assisted living administrators.

Chief Diversity, Equity & Inclusion Officer and Vice President of Clinical Services at AtlantiCare Health SystemCharisse Fizer, RN, MSN, FACHE, is vice president of Clinical Services and the chief diversity, equity and inclusion Officer (CDEIO) at AtlantiCare. The healthcare organization comprises AtlantiCare Regional Health Services, including AtlantiCare Regional Medical Center with three locations, Ambulatory Services and AtlantiCare Physician Group; the AtlantiCare Foundation, and AtlantiCare Health Solutions, an accountable care organization.
In her clinical role, Fizer leads the system’s healthcare delivery service lines. As CDEIO, Fizer leads the development of a transformative diversity strategy to align with the organization’s strategic plan and oversees the continuation and expansion of existing diversity, equity, and inclusion efforts. She is also responsible for creating new initiatives across the organization.
Fizer has been involved in many quality and performance improvement initiatives that have enhanced patient care. Under her supervision, AtlantiCare implemented many clinical programs, including transcatheter aortic valve replacement (TAVR); a treatment program for atrial fibrillation that involves both an electrophysiology program and CT surgery; and a new intensive care cardiac rehabilitation program. She was instrumental in AtlantiCare’s expansion of pharmacy services to include retail and community pharmacies, as well as pharmacy clinics to help assist patients taking anticoagulation medications.
Fizer joined AtlantiCare as the administrator for AtlantiCare Regional Medical Center’s Mainland Campus in 2006. She has also served as vice president of Clinical Operations and Ambulatory Services.
Fizer is involved in many community organizations and is a deaconess at Mt. Zion Baptist Church in Pleasantville, New Jersey. She is a member of the Atlantic City Chapter of The Links, Incorporated™ and immediate past president of its area Health Human Services Facet. She’s a Diamond Life member of Delta Sigma Theta sorority.
Fizer completed her Master of Science degree in health systems management at Vanderbilt University in Nashville, Tennessee, and a Bachelor of Science degree in nursing at Bradley University in Peoria, Illinois.
Fizer serves as a Health Services Board member for Northcentral University, a virtual university headquartered in San Diego, California, and on the Board of the Covenant House in Atlantic City. She is president of the South/Central New Jersey Chapter of the American Heart Association (AHA) Board and serves as a member of Eastern States AHA Board.
